Scheduling an Appointment

Payment must be made in full at the time of the appointment.

If you need an estimate of cost of services, please request prior to scheduling an appointment.

There are no payment plans or delayed billing options available.

Cash, check, all major credit cards, and Care Credit are acceptable forms of payment.

There will be a $30 penalty for any returned checks.

Please be aware of our following policies:

There is a $45 penalty fee for appointments cancelled with less than 24 hours notice.

If  no one is home when we arrive for a scheduled appointment (grace period of 15-20 minutes and attempt to call/email/text will be made), there is a $65 no-show penalty fee and the appointment will need to be re-scheduled.

Any outstanding account balance, including bounced checks, must be paid within 7 days.  Any balance over 30 days will incur 3% interest monthly.  If over 90 days due and no communication/response to our reminders, your account will be frozen and you will be terminated as clients of our clinic.  Your account will be forwarded to a collection agency for further actions.
